A long term condition (LTC) is defined as a condition that cannot at present be cured but can be controlled by medication and/or other therapies. Nationally, people with LTCs account for 50% of all GP appointments, 64% of all hospital outpatient appointments and over 70% of all inpatient bed days.[1]
